Protect your kids from the coming late-autumn and winter surge of CoVID and Flu viruses in our community with vaccinations! We have appointments open for Flu and CoVID vaccines. These are Nurse-only visits, no co-pay needed, and are very quick and efficient. Please call our office to schedule or ask any questions.
Vaccines strengthen our body's immune response to illnesses. While it is true that our immune system, without the help of vaccines, can often fight influenza (flu) or CoVID (after a typically miserable week or more of symptoms), success is not guaranteed and our immune systems, particularly for children, older adults, and those with chronic medical conditions, are not reliably victorious.- Every year, approximately 20,000 U.S. children are so sick with influenza that they need to be hospitalized. The U.S. loses roughly 200 children per year to influenza.
- Between January 1, 2020 and June 23, 2023 (late-2023 and 2024 data are not yet available) CoVID ended the lives of approximately 1,850 children in the US (about 530 US children died per year in that 3.5 year period).
PLEASE TALK TO PEDIATRICIANS AND/OR OTHER MEDICAL DOCTORS to address any questions you have about vaccines for both you and your children. There is so much disinformation and confusion about vaccines in the community and on the internet that an open-minded person can easily be misinformed or simply become exasperated with the subject. This often leads to a missed opportunity to protect you and your family with vaccinations. -

School Health Forms and Sports/Camp Participation Forms There will be a charge of $15.00 for the completion of a SINGLE health form by our office staff (NO CHARGE for WIC forms or working papers). The charge will be $30 for 2 or more forms. This charge will be applied to all forms sent in by mail or brought in to the office. We would appreciate payment prior to the completion of the form.School Health Forms and Sports/Camp Participation Forms
There will not be a charge for forms completed at a physical exam, IF they are brought to the office at the time of the appointment.
Due to the number of forms that we receive (especially before sports seasons and camp season), we cannot guarantee that they will be completed immediately. Please allow at least one to two weeks for us to complete forms. If you know that your child will be doing a sport, please contact the school well before the beginning of the season to obtain the forms.
If your child has not had a physical exam within one year of the date we receive the form, it cannot be completed by our office. Physical examinations can change, and if we are to certify that your child is in good health and without medical risk factors to play a sport, or attend camp, we need to perform a complete physical examination within one year prior to signing the form. Therefore, please schedule an exam well in advance of the date your child will be starting a sport or going to camp.
We strive to give our patients high quality care in a compassionate environment. By adhering to the above guidelines, we hope to make the process of health form completion a smooth one.
Thank you for your cooperation. -
RSV protection for infants (younger than 8 months old) Our office is currently providing our newborns and infants (under 8 months of age) protection against RSV (Respiratory Syncitial Virus) with an immunization called Beyfortus (AKA nirsevimab-alip). This is provided in a single injection which provides protection throughout an infant's first RSV season. It can be provided during a routine well-visit, including with other immunizations, or it can be provided at an appointment dedicated to it (please call to schedule). For more information, please discuss this at your well visits with us, orread more about Beyfortus by clicking here. If you would like to check your insurance coverage for this expensive immunization, use the code numbers 90380 and 90381 when discussing it with your insurer (almost every insurer completely covers the cost, but not all).
*When a mother receives RSV vaccine between 32 and 36 weeks of pregnancy, and at least 2 weeks before the baby's birth, that infant has sufficient protection against RSV in the first RSV season, and this immunization is not warranted. RSV protection for infants (younger than 8 months old)
